We encourage you to register as soon as you move into the area rather than wait until you are unwell, as records from your previous GP may take some time to be transferred to us.
You will need to fill in a form and a questionnaire to register. We need the name and address of your previous GP so that your records can be traced. At the time of registration we ask for ID for all adults aged 16+. One item should be a photo ID i.e. passport or driving license along with one document containing your address. This is to protect unauthorised people from gaining access to medical records.
You can register as a new patient using our online registration form. If you have not already registered with the National Health Service you will also need to complete a GMS1 which can be found here
Care professionals in England use an electronic record called the Summary Care Record (SCR). This can provide those involved in your care with faster secure access to key information from your GP record.
